Coordination of benefits
Coordination of benefits is the set of rules that decides which dental plan pays first when a patient is covered by more than one plan. The first plan is the primary and the other is the secondary, which considers what the primary already paid when it works out its own share.
Why it matters to the front desk
For a child covered by both parents, the birthday rule decides which plan is primary: the parent whose birthday falls earlier in the calendar year usually holds it. A secondary plan needs the same verification as a primary, with its active status confirmed on its own and its benefit detail actually pulled, not assumed because it is on file.
Secondary plans differ in how they pay. Under standard coordination, the secondary pays up to its own allowed amount minus whatever the primary paid, which can leave a small secondary payment. Under non-duplication of benefits, the secondary pays nothing when the primary's payment already meets or exceeds what the secondary would have paid as primary, the usual reason a secondary pays zero. Practices generally hold the patient's bill until the secondary has responded.
