Dental benefits verification checklist

A printable checklist of what to confirm with a dental payer when you verify a patient's insurance: the plan and its status, the deductible, the annual maximum, coverage percentages, frequency limits and waiting periods. Work through it for each patient, whether you check by phone, on a portal, or with software.

What to confirm for each patient

  1. Plan and status. Confirm the plan is active on the appointment date and note the effective date. Confirm a secondary plan the same way; do not assume it is active because it is on file.
  2. Network status. Note whether your office is in network or out of network for this plan.
  3. Deductible. Record the annual deductible and the amount remaining.
  4. Annual maximum. Record the annual maximum and the amount remaining.
  5. Coverage percentages. Record the percentage the plan pays for each category, such as preventive, basic and major, and for the specific procedures the patient needs.
  6. Frequency limits. Record how often each service is covered, for example one cleaning per six months. Ask which services share a limit: exams often share one count, cleanings and perio maintenance often share one count, and a full-mouth series and a panoramic image often share one clock.
  7. Frequency basis. Ask whether limits run on the calendar year or from the date of the last service.
  8. Waiting periods. Note any waiting period that applies to the treatment the patient needs.
  9. Downgrades. Ask whether the plan pays an alternate benefit, for example paying a composite filling at the amalgam rate.
  10. Missing tooth clause. Ask whether the plan excludes replacing a tooth that was missing before coverage began.
  11. Coordination of benefits. If the patient has two plans, confirm which is primary; for a child covered by both parents, the birthday rule usually decides. Ask whether the secondary pays standard coordination or non-duplication of benefits.
  12. Anything the payer did not return. Write down every item the payer left blank so the gap is visible, not assumed.
  13. Source and date. Record who you spoke to or which portal page you used, and the date of the check.
